It runs in a north-south direction starting from 165th Avenue and crosses State Highway 92, 167th Lane, Kendall Street, 168th Avenue, Kennedy Street, 173rd Avenue, 180th Avenue, Keokuk Street, Lowe Street, Merrill Place and 190th Avenue. It ends in a cul-de-sac and connects Lincoln Township, Otter Township and Belmont Township. County Road S23 is also known as 165th Avenue. County Road S23 also exists in White Breast Township and Palmyra Township.
