It runs in an east-west direction starting from West 5th Street and crosses North Main Street, North Hall Street, North Macomb Street, North Ray Street, North Valentine Street, North Government Street, North Haley Street, North Hugh Street and North Howe Street and ends at North Green Street. This street is part of the highways 12 and Outlaw Trail Scenic Byway. East 5th Street also exists in Madison County, Pierce County, Wayne County, Deuel County and Kimball County.
