It runs in a northeast-southwest direction starting from Moir Farm Road and crosses Delta Church Road, Northview Church Road, Phillips Road, Pitzer Road, Pleasant View Church Road and Len Alley Road and ends at Main Street. It connects Danbury and Stokes County. Sheppard Mill Road is also known as State Road 1652. This street is part of the Bicycle route "North Line Trace" as well as the Hiking trail "Mountains-to-Sea Trail: Segment 8". Sheppard Mill Road also exists in Pitt County.
