It runs in an east-west direction starting from State Route 132 and crosses Diamond Hill Road, Chestnut Lane, Jett Hill Road, Steep Gravel Drive and Wildlife Way and ends at Laurel Lindale Road. It connects Ohio Township and Monroe Township. Twelve Mile Road is also known as 12 Mile Road.
