It runs in a southeast-northwest direction and crosses Pearl Road, Walking Stick Ln, Woodin Road, Grand Army of the Republic Highway, Sisson Road, Claridon Troy Road, Chardon-Windsor Road, Stillwell Road, Mayfield Road, Headwater Lane, Ensign Road and Durkee Road. It ends in a cul-de-sac and connects Huntsburg Township, Middlefield Township, Middlefield Village, Claridon Township and Hambden Township. North State Avenue is also known as Old State Road. This street is part of the highway "608".
