It runs in a north-south direction starting from North Airport Road and crosses 233A Street, 234th Street, SD Highway 34, Southeast 10th Street, 235th Street, 238th Street, 240th Street and 242nd Street. It connects Lakeview Township, Madison and Franklin Township. 456th Avenue also exists in Leroy Township and Nunda Township.
