It runs in a north-south direction starting from 389th Avenue South and crosses 136th Street Southeast, 137th Street, 138th Street, 140th Street, 141st Street, 142nd Street and 144th Street. It connects West Rondell Township and Gem Township. This street is part of the highway "21". 389th Avenue also exists in Ordway Township, Garland Township, Brainard Township and Richland Township.
